From Internet Explorer - select File>Page Setup. A dialogue box like this image will appear. If there is any code in the "Header" or "Footer" boxes, make a note somewhere of what it is, (so you can restore it if necessary), delete it, then click on "OK". Then click on File>Print Preview to check that the code is indeed gone.
